Hi, the error says 404 which means the file could not be found. You might want to check your installation to see if there is anything blocking file uploads, and try to compare with other environments.

im using form grid to display multiple rows of image upload. images cant be shown immediately after form submit. i need to save as draft or submit form first in order to see the thumbnails in form grid . is it a bug or any workaround ?
here error on chrome dev console after submit popup form :
