Multi Approve - Error generating the data list

Hello,

I've tried to config "Multi Approve" as Map Activities to Form.

After "Submitted" I got this error message.

I've checked columns of "app_fd_gd_hr_apprv_trail_pd" table was generated at the first running process likely uncompleted.
